Knights of the Grand Cross wear a large cross and military trophy suspended from a wide black sash or baldrick which is placed over the right shoulder,crossing the breast and back diagonally and ending at the left hip.
The sash is worn under the waistcoat, except in the presence of a head of state, in this case, it is worn over the waistcoat. A large eight pointed or rayed silver star of rank, charged with a large cross, is also worn low on the left breast of the coat (see Figure 1).
Knight Commanders with Star wear a medium cross and military trophy around the neck, suspended from a black ribbon of medium width (see Figure 3a). A large eight pointed or rayed silver star of rank, charged with a small cross surrounded by a golden circle bearing a green laurel wreath, is also worn low on the left breast.

Knights wear a small cross and military trophy on a narrow black ribbon around the neck (see Figure 3). Alternately, Knights may wear the older badge type decoration, as a medal on the left breast of the coat. This is still a valid and traditional decoration, but it has recently been replaced by the neck ribbon style for ease in placement in the investiture ceremony.

It is appropriate to wear the miniature decoration and the rosette on less formal occasions. This miniature is a yellow and white gold papal bar of small size from which hangs a black silk ribbon and a miniature of the cross or star of appropriate rank. These may be worn on any social occasion but never worn together with the corresponding full-size emblem of the same rank.

It is now considered to be socially acceptable to wear a full-size insignia with Dinner Jackets (black tie). One neck decoration and a single star may be worn. The Grand Cross sash is not appropriate for any but Full Evening Dress (white tie). (See Figure 6).

Neither full-size insignia nor miniatures are appropriate for wear with business suits or ordinary daytime dress, although there is a small flat lapel badge, or, alternately, a rosette, both of which display a small cross of the Order and an indication of rank, which Knights are encouraged to wear on the lapel at all times.
